Our community nursing team visited the participant at home three times a week to manage medication, wound care and mobility exercises. Working closely with the family and GP, we built a simple routine that improved independence and confidence within the first month, and every visit was recorded so the family always knew what had changed and what came next.
The project combined in-home nursing, allied health input and support coordination. Progress was reviewed monthly and the plan adjusted as goals were achieved, so funding was always used where it made the biggest difference.
